How do ChatGPT, Gemini, Google AI Mode and Perplexity decide which clinic to mention?

Medslots team, Letex Media Co.How we write and check facts

Updated 13 min readFacts checked

Short answer

ChatGPT, Gemini, Google AI Mode and Perplexity don't hold a separate database of clinics. They read your Google Business Profile, Practo, JustDial, your website and any other page that names you, then repeat whichever facts appear most consistently. Getting these details right and matching everywhere helps far more than any markup trick.

Key takeaways

  • AI tools don't have a private list of clinics. They lift facts from your Google Business Profile, Practo, JustDial, your website and other pages that mention you.
  • ChatGPT and Perplexity crawl the web with their own bots; Gemini and Google's AI Overview and AI Mode draw on the same Search index Google already builds.
  • The clinic controls its own listings and website. It does not control, and cannot pay for, a place inside any tool's answer.
  • Google says no special schema or AI text file is needed to appear in AI Overviews or AI Mode. Consistent, matching facts across every listing do far more.
  • Fake reviews, a keyword-stuffed clinic name and promises of a guaranteed place in an AI answer all break a real rule and tend to backfire.

Some patients now ask an AI tool which clinic to visit, instead of typing a search and scrolling a list of links. This guide covers how ChatGPT, Gemini, Google AI Mode and Perplexity actually decide what to say about a clinic, what is genuinely in your hands, and what to leave alone.

None of these tools have a phone number for your clinic or a form you fill in. They read what already exists about you on the web and repeat what they find most consistently. That is the idea behind everything below.

How do ChatGPT, Gemini, Google AI Mode and Perplexity find out about a clinic?

Each tool pulls from ordinary listings and pages, not a special medical directory. Your Google Business Profile, your Practo or JustDial listing, your own website and any other page that names your clinic are the raw material. When those sources agree, a tool has a clear fact to repeat. When they disagree or say nothing, it usually leaves your clinic out rather than guess.

Where an AI answer's facts about a clinic come fromFour source boxes on the left: Google Business Profile; directory listings such as Practo and JustDial; your clinic's website; and other mentions such as news or associations. Arrows from all four converge on a middle box, consistent facts across every source. An arrow from that box leads to a final box, named correctly in the answer.Google Business ProfileHours, phone, categoryDirectory listingsPracto, JustDial and othersYour clinic's websiteText a crawler can readOther mentionsNews, associations, socialConsistent facts across every sourceNamed correctly in the answer
A clinic that is missing from every source on the left has nothing for a tool to repeat. A clinic whose sources disagree often gets skipped rather than named with the wrong details.

This is also why fixing your Google Business Profile, your Practo listing and your website is not four separate jobs. It is one job: make the same true facts appear everywhere a tool might look.

How are the four tools different?

The four names get used loosely, but they are not built the same way. What follows is only what each company says about itself, kept plain.

ChatGPT, Gemini and Google's AI answers, and Perplexity

This table states only what each has said in writing about how it finds and reads pages.

AI Overview and AI Mode are not the same feature. For how they differ from each other and from an ordinary Google search, read our post on Google AI Overviews and AI Mode for clinics.

Could your website already be blocking the bots that make you visible?

Sometimes a clinic's website is blocking the exact bot it needs. ChatGPT, Perplexity and Google each publish more than one bot, for different jobs, and a website can block one without blocking the others. A developer who added a "block AI scrapers" rule to protect content can end up blocking the same bot that would have shown the clinic in an answer.

Which bot does which job, and what blocking it actually does
BotCompanyWhat it doesIf it is blocked in robots.txtSource
GPTBotOpenAICrawls pages that may be used to train future modelsStops the page being used for training. Does not remove the clinic from ChatGPT's current search answersOpenAI (opens in a new tab)
OAI-SearchBotOpenAICrawls and indexes pages so ChatGPT's search feature can find and cite themThe page will not be shown or cited in ChatGPT's search answersOpenAI (opens in a new tab)
ChatGPT-UserOpenAIFetches one page only when a user asks ChatGPT to open itRarely blocked on purpose. OpenAI says it is not used for automatic crawlingOpenAI (opens in a new tab)
PerplexityBotPerplexityBuilds Perplexity's own search indexThe page drops out of that index and stops appearing in answersPerplexity (opens in a new tab)
Perplexity-UserPerplexityFetches a page only while answering one user's specific questionPerplexity's own docs say this fetcher "generally ignores robots.txt rules", so blocking it here mostly has no effectPerplexity (opens in a new tab)
GooglebotGoogleThe same crawler behind ordinary Search, AI Overviews and AI ModeThe page drops out of Search entirely, AI Overviews and AI Mode includedGoogle (opens in a new tab)
Google-ExtendedGoogleControls whether crawled content trains future Gemini app modelsDoes not remove the page from Search, AI Overviews or AI ModeGoogle (opens in a new tab)

Each company treats its bots as independent settings. Allowing one and disallowing another, on the same site, is normal and does exactly what it says.

To check your own site, type yourclinic.com/robots.txt into any browser. A line disallowing OAI-SearchBot, PerplexityBot or Googlebot means that tool cannot read your pages at all. A line naming only GPTBot or Google-Extended affects training, not whether a patient sees the clinic in an answer.

Where do these tools actually get their facts about your clinic?

Five places hold almost everything an AI tool could say about your clinic. Three of them you can claim and edit directly, for free.

What can a clinic actually control?

Everything you can fix costs time, not money, and none of it is a trick. This is the short version; each item links to the fuller guide.

In a clinic's own hands

  • Claim and complete your Google Business Profile.

Name, category, hours, phone and photos. See our guide to Google Business Profile for doctors.

  • Keep Practo and JustDial accurate, and close listings you no longer use.

  • Put services, fees and timings on your website as text.

Not locked inside a photo or a PDF a crawler cannot read.

  • Use the exact same clinic name, address and phone number everywhere.

  • Ask every patient for a Google review the same way, with nothing offered in return.

  • Test what AI tools currently say about your clinic, and recheck monthly.

  • Open yourclinic.com/robots.txt and check it does not block OAI-SearchBot, PerplexityBot or Googlebot.

Blocking GPTBot or Google-Extended alone only stops training. It does not remove a clinic from an AI answer.

Google's own advice on local ranking says relevance, distance and prominence decide what a patient sees, and that there is no way to request or pay for a better ranking. See Google's page on local ranking (opens in a new tab). What is not in your hands: any tool's exact formula, and any paid way to sit inside an answer. OpenAI says ads shown in ChatGPT are labelled, kept separate from its answers and do not influence what ChatGPT says.

What is the step-by-step way to become visible?

There is no shortcut that skips these steps, only an order that gets the biggest, free fixes done first.

  1. Step 1: Fix your facts once

Decide the exact clinic name, address, phone and services you will use everywhere, starting today.
  2. Step 2: Claim your Google Business Profile

Add or claim it at business.google.com, then verify it. Free.
  3. Step 3: Claim Practo and JustDial

Both offer a free basic listing. Match every detail to your Google profile.
  4. Step 4: Make your website state the facts as text

Services, fees, timings and address, readable without opening an image or a PDF.
  5. Step 5: Build genuine outside mentions

Ask patients for reviews the honest way. Local press, associations and directories help too, but only if the mention is real.
  6. Step 6: Test, then recheck monthly

Ask ChatGPT, Gemini, AI Mode and Perplexity the questions a patient would, and note what changes.

The website and directory steps above each deserve more detail: our checklist of clinic website must-haves in India and our comparison of a Practo or JustDial profile vs your own clinic website go through each in full.

How do you check what these tools currently say?

Open each tool on your own phone and ask what a patient would: the name of your specialty and area, your clinic's name directly, and a common service. Note whether you are named, whether the details are right, and which websites get cited. Answers change with location and the day, so treat one check as a snapshot, not a verdict.

What should a clinic never try?

Each shortcut below breaks a real, written rule, and tends to backfire rather than help.

Do schema markup and llms.txt actually matter?

Not on their own. Google's own developer documentation for AI features says plainly that no special schema.org markup and no new AI text file are needed to appear in AI Overviews or AI Mode. The requirement is the same as ordinary Search: your page has to be crawlable and readable as plain text.

  • Crawlability and plain text beat markup. If a bot cannot fetch your page, or your services sit only inside an image, no tag fixes that.
  • Consistency beats a clever file. The same name, address and phone everywhere does more than any single addition to your website's code.

We took this question further in its own post, including what Google, OpenAI and Perplexity each say about llms.txt and the specific FAQ-schema change in Google Search: read does schema markup help a clinic show up in ChatGPT?

Common questions

Does Google AI Mode work differently from a normal Google search for a clinic?

Yes. AI Mode is a more conversational way to search inside Google, built to pull together a longer answer instead of a list of links. Google added it to Search in India with English and Hindi in June 2025, and later added Bengali, Kannada, Malayalam, Marathi, Tamil, Telugu and Urdu. A patient may see a very different answer in AI Mode than in an ordinary Google search for the same words.

Do Practo and JustDial charge to list a clinic?

A basic listing is free on both. Practo's own help pages say the listing on Practo comes at no cost and is separate from any paid Practo product. JustDial's own free-listing page says the same. Either platform may also sell paid promotion on top, which is a separate choice.

How often do ChatGPT, Gemini and Perplexity update what they know about a clinic?

None of the four publish a fixed schedule, and it isn't the same for every clinic. The practical approach is to fix your facts everywhere they're wrong, then check again in a few weeks rather than expecting a same-day change.

Can I add words like "best" or "No. 1" to my clinic's name to stand out in AI answers?

No. Google's own guidelines for representing a business do not allow marketing taglines or superiority claims inside the name field, whichever tool reads it. The 2002 NMC code also treats self-advertisement by a doctor as unethical, and the ASCI code expects a claim like best to be backed by evidence. Use the name on your signboard everywhere.

If my website blocks GPTBot, will that stop my clinic appearing in ChatGPT?

Not by itself. OpenAI's own documentation treats GPTBot, OAI-SearchBot and ChatGPT-User as independent settings, so blocking GPTBot only stops a page being used to train future models. The bot that lets ChatGPT's search feature find and cite a page is OAI-SearchBot, and that is the one that decides visibility.

AI tools mein mera clinic kaise dikhega?

Koi paid shortcut nahi hai. Google Business Profile, Practo aur JustDial par apna naam, address aur phone number bilkul same rakhein, apni website par services aur timings text mein likhein, aur genuine tareeke se Google reviews maangein. Phir ChatGPT, Gemini aur AI Mode mein khud check karein ki clinic ka naam sahi aa raha hai ya nahi.
