How does Perplexity find and cite clinics, and what can you actually influence?

Medslots team, Letex Media Co.How we write and check facts

Updated 10 min readFacts checked

Short answer

Perplexity answers by searching the live web and citing the pages it used. Some cited domains carry a Government, Academic or Trusted label, decided by Perplexity's own review, never by payment. There is no Perplexity listing to claim. What you can influence is your website's own text and how consistent your details are across the web.

Key takeaways

  • Perplexity reads the live web for each question and shows numbered citations. Its Help Center describes this as searching, summarising and citing, not answering from memory.
  • Some cited domains carry a Government, Academic or Trusted shield. Perplexity says this rates the whole website, not one page, and that payments or partnerships never decide it. Most sites, including almost every clinic website, carry no label at all, and that is not a penalty.
  • There is no Perplexity business profile to claim, unlike Google Business Profile. Be wary of anyone offering to get your clinic a source label or a guaranteed mention for a fee.
  • An academic study (Columbia Journalism Review's Tow Center, March 2025) found Perplexity gave the most accurate citations of 8 AI search tools tested, and still got 37% of them wrong. Always click through and check what it actually says about you.
  • Many popular 'Perplexity SEO' tips are not documented anywhere by Perplexity, including specific claims about page counts, citation pipelines and citation timelines. One claim that does check out: PerplexityBot reads a page's plain HTML and does not run JavaScript, so facts hidden behind a script may go unseen.

A patient in your city asks Perplexity which skin clinic to visit for acne treatment. What decides whether your clinic gets named, and is there anything you can actually do about it? Here is what Perplexity itself says, not what an SEO seller says it says.

How does Perplexity find an answer?

Perplexity does not answer from memory. Its own Help Center says it "searches the internet in real-time, gathering insights from top-tier sources" for every question, then compiles the most relevant parts into an answer with numbered citations linking back to where each fact came from.

  1. Step 1: Reads the question

Perplexity works out what is actually being asked, including local details like a city or specialty.
  2. Step 2: Searches the live web

It fetches current pages, not a fixed answer written in advance, gathering from articles, websites and directories.
  3. Step 3: Summarises what it finds

The most relevant facts are pulled together into one answer, in plain conversational language.
  4. Step 4: Adds numbered citations

Each answer links to the pages it used, so a reader can open the source and check it directly.

Two crawlers do the fetching: PerplexityBot keeps a search index of the web, and Perplexity-User visits a specific page only when it is needed to answer a live question. Perplexity's own developer documentation says neither one is used to train its underlying AI models, and that a site can block PerplexityBot in its robots.txt file if it chooses to.

What are the shield icons next to some sources?

Some citations in a Perplexity answer carry a small shield icon marked Government, Academic or Trusted. Perplexity's Help Center is specific about what this does and does not mean, and it is the one part of how Perplexity works that almost no clinic-marketing page explains correctly.

What a Perplexity source label actually covers
QuestionWhat Perplexity says
Is the label for one article, or the whole site? The whole website. A single page is never labelled on its own.
Can a clinic pay for a label? No. Perplexity states that its partnerships, payments and other business arrangements do not affect a site's label.
Does no label mean a site is low quality? No. Most of the web, including almost every individual clinic website, has no label at all. That is not a penalty.
What earns a label? Objective checks such as whether the site corrects mistakes, credits its authors, and keeps news separate from advertising.

Source: Perplexity Help Center: Understanding source labels (last modified 9 Sep 2026) (checked 17 Sep 2026).

In practice this means your own clinic website will almost certainly never carry a shield, and chasing one is not a realistic goal. The labels matter more for the sites Perplexity cites about your clinic, such as a government health directory or a well-known publisher. A directory or review site that already carries a Trusted label is a more useful place to be listed correctly than a new page on your own site written only to look "AI-friendly".

How is Perplexity different from Google and ChatGPT?

All three now show AI-written answers, but they are built differently, and each documents itself differently too. This is the short version; our guide to AI answers for clinics covers Google and ChatGPT in more depth.

The practical difference for a clinic: on Google, claiming and correctly filling in your Business Profile is a direct, well-documented lever. On Perplexity, there is nothing to claim. Your only lever is the same open web that Perplexity already reads, which is also true for ChatGPT.

How accurate are Perplexity's citations?

Better than several other AI search tools, but still wrong a meaningful share of the time. This is not a clinic-specific figure, and no clinic-focused study of this kind exists yet, but it is the clearest sourced data point on how far to trust any AI-cited answer.

Share of citations found incorrect, 8 AI search tools tested on verbatim news excerpts (Mar 2025)Columbia Journalism Review's Tow Center gave 8 AI search tools 1,600 verbatim excerpts from real news articles and asked each to identify the correct headline, publisher and URL. Shown here because it is the most rigorous, dated, independent test of AI citation accuracy we found, and Perplexity came out ahead of the other tools tested.

The lesson for a clinic is simple: a Perplexity answer that names you, or a competitor, is not guaranteed to be accurate even when it looks confident and cites a source. Always open the cited page yourself before you act on what an AI answer says, about your own clinic or anyone else's.

Which "Perplexity SEO" tips are actually documented?

Search for how to get cited by Perplexity and you will find dozens of confident guides, almost all written by marketing agencies, not by Perplexity. Some of the advice is genuinely useful. Some of it is a specific-sounding number nobody can point to a source for. We checked four claims that keep coming up against what Perplexity itself publishes, and against one independent look at crawler behaviour.

Common "Perplexity SEO" claims, checked against the source
What agencies commonly sayWhat the source actually says
Add an llms.txt file to your website and Perplexity will use it to decide what to cite.Perplexity's own crawler documentation opens by pointing readers to its documentation index at docs.perplexity.ai/llms.txt, so Perplexity clearly uses the format for its own developer docs. Nowhere on that page, or in its Help Center, does Perplexity say it reads an llms.txt file on a clinic's or any other website to decide what to cite.
Perplexity checks about 10 pages per question and cites 3 to 5 of them, following a multi-stage pipeline.Perplexity's Help Center describes searching, summarising and citing in general terms. It does not publish a page count, a stage count or a source count for how many pages it checks or cites.
A new or updated page gets cited within 1 to 7 days.Perplexity does not publish how often it re-indexes a given page. Its Help Center says it searches the live web for each question, not that every page refreshes on a fixed schedule.
A JavaScript-heavy website is invisible to Perplexity. This one checks out. An independent analysis of crawler traffic found PerplexityBot fetches a page's raw HTML and does not execute JavaScript, so content that only appears after a script runs may not be read at all.

Where no such statement exists there, we say so rather than guess.

The practical point: a service that promises a guaranteed citation timeline or a guaranteed llms.txt fix is selling something Perplexity does not document. A JavaScript-dependent page is different: it is a real, checkable problem. On a computer, right-click your homepage and choose "View Page Source"; if your address, phone number, fees and hours are not visible there as plain text, a script is likely adding them after the page loads, and Perplexity may never see them. On a phone, ask whoever built or manages your website to check the same thing for you.

What can a clinic actually influence?

Nothing here is Perplexity-specific advice sold by an agency. It follows directly from how Perplexity says it works: it reads the open web live, so the same basics that help Google and ChatGPT also help here.

  • The same details, everywhere. Your clinic name, address, phone number and services, written identically on your website, your Google Business Profile and any directory that lists you. Perplexity retrieves from all of these, and disagreement between them is a common reason a real clinic gets skipped.
  • A website that states facts in plain text, not only in JavaScript. A page that says your hours, fees and services directly, in ordinary sentences, is easier for Perplexity to quote accurately than a page built mostly of images or a PDF, and this matters even more when a script adds the text after the page loads: PerplexityBot reads a page's raw HTML and does not run JavaScript, so anything added afterwards by a booking widget or script may go unseen. See the check above.
  • A presence on the directories Indian patients and Perplexity actually use. Most "get cited by AI" guides are written for the US market and point clinics toward Zocdoc or Healthgrades, which barely exist here. One Indian agency's own test of 4 AI engines (opens in a new tab) found Practo, Google Business Profile, Justdial and Lybrate listings being cited for Indian clinic questions instead. A correct, complete profile on these, not a US directory, is the more useful hour to spend.
  • Genuine reviews, not managed ones. Google's own guidance (opens in a new tab) and ASCI's advertising code (opens in a new tab) both treat fake or incentivised reviews as a violation, and a thin or inconsistent review history is unlikely to read as a strong signal to any AI tool either.

None of this guarantees a mention. Perplexity does not publish a ranking formula, and no credible source, including Perplexity's own documentation, claims one exists.

How do I check what Perplexity says about my clinic?

This takes about five minutes on your phone. Do it the same way a patient actually would: as a stranger who does not already know your clinic's name.

A 5-minute Perplexity check for your clinic

  • Open Perplexity and ask a question the way a patient would.

For example, "best [your specialty] clinic in [your area]" or "which clinic treats [a condition] near [your area]".

  • Note whether your clinic is named at all.

  • If it is, check every detail against the truth.

Address, phone number, timings and services, exactly as they are today.

  • Open every cited link.

See which of your own pages, or which other sites about you, Perplexity actually used.

  • Ask the same question again in a day or two.

AI answers can change between sessions; one check is a snapshot, not a verdict.

If your clinic is missing or wrong, the fix is not a Perplexity-specific trick. It is the same list above: fix your details everywhere at once, and give the open web something plain and correct to find. Run the same kind of check on ChatGPT and Gemini too; our 5-minute ChatGPT check walks through that side.

What should a clinic not do?

  • Don't pay for a promised mention or label. Treat any such offer the way you would treat a promise to buy a place inside ChatGPT's answer: with real doubt.
  • Don't let your details drift out of sync. A phone number or address that changed on one site and not another is one of the simplest reasons an AI tool skips a real clinic. One Indian agency's own test of 4 AI engines (opens in a new tab) observed this pattern too, though it is that agency's own, non-independent work, so treat it as indicative rather than proof.
  • Don't assume a Perplexity answer about you is correct. Run the 5-minute check above every so often, the same way you would check a listing on any directory.
  • Don't write pages only for AI tools. A page that states your facts plainly for a patient reading it is the same page that is easiest for Perplexity, Google and ChatGPT to quote.

Common questions

Can I pay to get my clinic a source label or a mention on Perplexity?

Not through anything Perplexity documents. Perplexity says its source labels come from an independent review of a whole website and that partnerships or payments do not affect them. We know of no way to buy a place inside a Perplexity answer. Be wary of anyone who promises one.

Does my clinic need a Perplexity business listing?

There is no such listing to create. Unlike Google Business Profile, Perplexity does not offer a page for a business to claim or edit directly. Its answers are built from what already exists on the open web, including your website, your Google profile and other sites that mention you.

Why does Perplexity mention a nearby clinic but not mine?

Usually because the other clinic's name, address and phone number are stated the same way everywhere it appears, and its website answers the question in plain text Perplexity can quote directly. Missing, outdated or inconsistent details are the most common reason a real clinic gets left out.

Is Perplexity's answer about my clinic always correct?

Not necessarily. An independent study found even Perplexity, the most accurate of 8 AI search tools tested on citing news articles correctly, still got 37% of its citations wrong in March 2025. Treat any AI answer about your clinic as something to verify, not as a fact.

Does Medslots check what Perplexity says about my clinic?

Medslots checks whether ChatGPT and Gemini mention your clinic and keeps your Google Business Profile current. It does not currently include a Perplexity-specific check. Ask us on WhatsApp if you want to know more.

Will adding an llms.txt file to my clinic's website get it cited by Perplexity?

There is no evidence of that. Perplexity's own crawler documentation points readers to an llms.txt file for its own developer docs, so Perplexity clearly knows the format, but nothing in that page or its Help Center says it reads an llms.txt file on a clinic's website to decide what to cite. Treat it as unproven, not a fix worth paying someone for.

Update log

  • : published.
  • : expanded with a myths-vs-documentation table for common 'Perplexity SEO' claims (llms.txt, citation counts and timelines, JavaScript rendering), localised the directory advice to Practo, Google Business Profile, Justdial and Lybrate instead of US platforms, and added one FAQ on llms.txt.
