Missed calls at your clinic: how to count them and follow up on WhatsApp

A missed call is an easy way to lose a patient without noticing. Nobody complains, nothing gets written down, and the caller may simply go somewhere else.

Why does a missed call cost more than it looks?

Because the clinic never finds out what the caller wanted. A patient who gets no answer does not leave a message saying "I will go to the next clinic now". They just call the next number on Google Maps or Practo, and your front desk sees one more missed call among many.

Calls go unanswered for ordinary reasons. The receptionist is billing a patient at the counter. The phone rings during the evening rush. Someone calls at 9 pm after the clinic has shut, or during the lunch break. None of this means your staff are careless. It means one person cannot hold a phone and a queue at the same time.

A few fixes that don't need any software

Some missed calls are simply the queue winning. A few small changes at the front desk catch some of these before they ever need a follow-up.

Small fixes worth trying first

  • Give the phone to one person as their first job during the rush, not their fourth.

    If the same person is also billing and giving directions, the phone loses every time there is a queue.

  • Check whether call waiting is silently dropping a second caller.

    On some phones and networks, a second call while the first is on hold goes straight to a busy tone instead of ringing.

  • Keep a second handset or a Bluetooth earpiece within reach of whoever is free.

    So the nearest free person can pick up even while walking a patient to the door.

  • List your calling hours next to your number on Google Maps and WhatsApp.

    Some after-hours calls are simply patients who did not know the clinic was closed.

Not every missed caller was a new patient. Some wanted directions, some were existing patients asking about a report, and some were sales calls. That is exactly why counting matters: you need to know your own mix before you spend a rupee fixing it.

How do you count missed calls for 7 days?

Keep a simple sheet next to the clinic phone for one normal week, and fill it in from the call log at the end of each day. Print the sheet below, or copy the columns into a notebook.

How to use the sheet

  1. Pick a normal week. Avoid a week with a festival holiday or the doctor on leave, or your numbers will not reflect a usual week.
  2. Fill it in once a day, at closing time. Open the call log on every phone the clinic uses for patients, including the doctor's own if patients call it.
  3. Count each number once a day. If the same person called three times in a row, that is one caller who tried hard, not three callers.
  4. Write "rush hours" the way your clinic defines them. For many clinics that means the morning and evening OPD peaks.
  5. Fill the last two columns honestly. "Called back same day?" and "Booked?" are the columns that show what the missed calls turned into.

What counts as "missed": unanswered, busy and after hours

Count a call as missed if the caller did not speak to anyone at the clinic. That includes three kinds.

What to count as missed

  • Count it if the phone rang and nobody picked up.

    Also count calls that were cut after one or two rings while staff were busy.

  • Count it if the caller got a busy line or a waiting call you could not take.

    Some of these may not show in the call log at all, depending on your phone and network settings. So treat your total as the least, not the most.

  • Count it if the call came when the clinic was closed.

    Lunch break, after closing, Sundays and holidays. Mark these in the after-hours column.

  • Leave out calls you know were not patients.

    Sales calls, suppliers and wrong numbers. If you are not sure, count it.

How this is worked out: A month is taken as 4.3 weeks (52 weeks divided by 12). Monthly estimate = likely new patients in your week × 4.3 × your first-visit fee. It counts only the first visit, not any follow-up visits or treatment. Nothing you type here is saved or sent anywhere.

Look at the answer as a ceiling, not a target. If the monthly figure is small, missed calls may not be your clinic's biggest problem, and that is useful to know too. There are three: staff call back from a list, staff send a WhatsApp message by hand, or software sends the follow-up automatically. All three work if they happen the same day. They differ in who has to remember and what happens at night.

A same-day routine that works without software

If you follow up by hand, a fixed routine matters more than speed on any one call.

  1. Follow up the newest missed calls first

    Send the short WhatsApp message below or call back. Tick the number off on the sheet.
  2. Go through the call log once, top to bottom

    Anyone missed during the rush gets their follow-up now, not at closing time.
  3. Call anyone who has not replied

    One phone call is enough. Do not send a chain of reminder messages.

Say who you are, that you saw their call, and ask how you can help. Nothing more. Two or three lines are enough, and the caller should be able to answer in a few words.

Notice what the clinic does not do in that chat: it does not ask about the pain or suggest anything for it. Questions about symptoms go to the doctor, on a call or at the visit.

What to leave out

  • Offers and discounts. A missed call is not permission to advertise. More on this in the rules section below.
  • Health details. No diagnosis, test names or treatment in the first message. The phone may belong to a family member.
  • Pressure. Skip lines like "only two slots left". Offer times and let the caller choose.
  • A chain of messages. One message, then one call if there is no reply. After that, leave it.

What about calls and messages after hours?

Calls and WhatsApp messages that arrive at night need different handling, because the tools that help with one do nothing for the other.

For WhatsApp messages, the WhatsApp Business app has an away message that replies automatically to messages received outside your business hours. It tells the patient when you will be back. For phone calls, the away message does nothing: it answers messages, not calls. A patient who calls at 9 pm and gets no answer hears nothing from the clinic until someone follows up. That is why after-hours calls get their own column on the sheet and their own slot in the morning routine.

Can you WhatsApp someone who only called?

Do it carefully, if at all: message only the number that contacted you, only about their call, and stop the moment they ask. When in doubt, a phone call back is the safer choice.

A missed call gives you the number, but not a clear "yes, message me". That is why the follow-up should read like a reply to their call, not like a marketing message, and why a single short note is the limit. If the caller does not reply, call them. If they say they did not mean to call, apologise and remove the number from your list.

What about calling back instead of messaging?

Calling back avoids the WhatsApp consent question entirely, and it is unlikely to run into India's telecom "Do Not Disturb" rules either, even if the number is registered on the National Customer Preference Register.

This changes the moment the call turns into an offer for something the caller did not ask about. That becomes a promotional call, and promotional calls need consent and a registered template whether or not the number is on DND. Keep the callback about their call, the same rule as the WhatsApp message above.

If your clinic uses WhatsApp software rather than the phone app

Tools built on Meta's WhatsApp Business Platform follow an extra rule. Meta's documentation says a 24-hour customer service window opens when a WhatsApp user messages you, and ordinary messages can only be sent inside that window. Outside it, only pre-approved template messages can be sent. A phone call is not a WhatsApp message, so a first follow-up to a caller who has never written to you goes out as a template. Common questions

Should the doctor call back or send a message?

Either works, as long as it happens the same day. The first follow-up does not need the doctor: the front desk can send a short WhatsApp message or call back. A phone call suits older patients and anyone who sounded worried. The doctor steps in when the caller has a clinical question.

How quickly should we follow up?

The same day, and during clinic hours as soon as the front desk is free. We could not find a reliable Indian study on the ideal time, so we will not quote one. The simple test: reach the caller before they have had time to book somewhere else. Calls that came after closing get a follow-up when the clinic opens.

Can we send a WhatsApp message to someone who only called?

Be careful. WhatsApp's Business Messaging Policy says a business may contact people who have given it their number and agreed to receive its messages on WhatsApp. A missed call gives you a number, not that agreement. If you do message, send one short note about their call only, and stop if they ask. If in doubt, call back instead. This is not legal advice.

Will calling back a missed caller break DND (Do Not Disturb) rules?

Not in the ordinary case. TRAI's telecom rules stop "commercial communication": a call or message whose main purpose is to advertise or solicit business, sent without the recipient's consent. A short callback that answers a call the patient made to you is not advertising anything, so it does not fit that definition in the first place. This changes the moment the call turns into an offer for something they did not ask about. This is not legal advice.

What if the caller was an existing patient?

Check the number against your appointment book or patient register before you reply, so you can greet them properly. Still keep treatment details out of the first message, because a family member may be using that phone. If they want to discuss a report or a symptom, pass it to the doctor.
