Why the sources disagree

The same job can show ₹13,003 on one site and about ₹34,950 on another, with PayScale's own number sitting at about ₹17,393. Three things explain most of the gap.

  • Sample size. Indeed's receptionist page rests on 798 salary reports. Its medical receptionist page rests on 25, and PayScale's receptionist figure rests on just 37 profiles, so a handful of entries can move any of them a lot.
  • Job title. "Healthcare/hospital receptionist" and "receptionist at a two-doctor clinic" are not always the same job, even when the title looks similar.
  • Data and date. Indeed shows salaries reported to it. Salary.com uses its own salary data. Dates range from April 2024 to May 2026.

So the number that matters is what receptionists are paid in your own town. Use the table as a range to check against, not as your answer.

Is salary the whole cost of a receptionist?

No. Salary is the biggest line, but a hire costs more than the payslip shows. None of this is a reason not to hire. It is just what belongs in an honest comparison.

  • Employer contributions, if the scheme applies. Where ESI applies, the employer pays 3.25% of the employee's wages, according to ESIC's contribution page (opens in a new tab) (about ₹472 a month on a ₹14,523 salary). Whether ESI or PF applies to your clinic at all is a separate question, covered just below.
  • Leave. Weekly offs, festivals and sick days. Someone else has to sit at the desk, or the phone rings with nobody to pick it up.
  • Training. The first weeks go into learning your fees, timings, the doctors' schedules and how you want patients spoken to.
  • Replacing someone who quits. Notice period, finding someone new, training again. There is no reliable India-wide figure for how often clinic receptionists leave, so count your own: how many have you hired in the last three years?

Does ESI or PF actually apply to your clinic?

Not automatically. Both schemes carry a headcount test, and a lot of small clinics fall under it without realising.

The Code on Social Security, 2020 took effect on 21 November 2025 and brought ESI and PF under one law, but it did not change either threshold. Count your own staff, doctor included, before you budget for either cost on a new hire, and ask your accountant to confirm what applies in your state.

A receptionist on an 8-hour shift, six days a week, covers 48 of the 168 hours in a week. A WhatsApp AI receptionist replies at any hour, including late evenings and Sundays when the desk is closed.

The two also cover different work. Your receptionist looks after the patient standing at the counter. On WhatsApp, Meta has charged businesses per template message delivered on the WhatsApp Business Platform since 1 July 2025, according to Meta's WhatsApp pricing page (opens in a new tab). Ordinary replies inside an open customer service conversation are free; the charge is limited to template messages sent outside that window, plus a marketing-category rate that rose again for India from 1 January 2026. When you compare vendors, ask whether their price includes those charges or bills them on top.

  • Take payments at the desk.

    A chat should tell the patient to call the clinic or go to the nearest hospital, never try to manage it.

  • Answer clinical questions.

    Those belong with the doctor.

So the useful question is often not "receptionist or software". It is whether the hours your desk cannot cover are costing you enquiries, and what covering them is worth to you.

How this is worked out: Hours covered = hours a day × days a week × 4.33 weeks in a month. Cost per covered hour = salary ÷ hours covered. Hours nobody is at the desk = about 727 hours in an average month (24 × 7 × 4.33) minus hours covered. Nothing you type here is saved or sent anywhere.

Does ESI or PF apply to a small clinic's front desk staff?

Not automatically. ESI generally needs the clinic to be in an area where the state has notified private medical institutions, employing 10 or more persons, and the employee to earn up to ₹21,000 a month. PF registration becomes mandatory only once a clinic employs 20 or more persons. Many small clinics fall under both thresholds, though you can register voluntarily. Ask your accountant to confirm for your clinic and state.
