What's the checklist for opening a second branch of your clinic in India?

Most checklists for a second clinic branch read like a shopping list: get this licence, get that one. Two things they usually skip: almost none of this paperwork carries over from branch one, because it attaches to the premises, not the doctor or the business, and some of it is the same rule everywhere in India while some of it changes by state, which decides who you actually have to call.

Which registrations and licences does a second branch need?

Start by sorting what you already have from branch one into two piles: what stays the same, and what has to be done again for the new address. The registration number stays the same. Almost everything else attaches to the premises, so it doesn't.

This matters beyond neatness. Where a registration is tied to the premises, running an unregistered branch can put its right to operate at risk, separate from any question about the doctor's own standing. The table below lists what to check before the new branch opens. Whether each one is the same rule everywhere or changes by state decides whether you need a form or a phone call to a local office.

Does the Clinical Establishments Act apply the same way to a new branch?

Only in the states and union territories that adopted it. Where a state didn't adopt the central Act, it usually runs its own clinical establishment or nursing home registration law instead, with its own application and its own rules, not the same set. Either way, the registration is tied to the address, so opening branch two in a state that adopted the Act doesn't extend branch one's registration to cover it.

Does the second branch need its own Google Business Profile?

Yes. Google's guidelines call for one profile per real business location, with a phone number and website that represent that specific address rather than a shared central number or a generic homepage. A single listing cannot cover two addresses, however closely the two branches are run.

This is worth getting right early, because the listing takes time to verify and you don't want it holding up the rest of the launch. Should the new branch have its own phone number and WhatsApp?

Yes, for the same reason as the Google profile. Google's guidelines prefer a local number that connects directly to that specific location over a shared central helpline. A WhatsApp Business number is also tied to one account, so if the branch gets its own WhatsApp presence, that number has to be verified as its own profile, not added to branch one's account.

  • One number that patients actually reach at that branch, not a switchboard that then has to transfer the call.
  • A separate WhatsApp Business profile for the branch's number, with its own hours, address and catalogue if it has different services or fees.
  • A plan for who answers in the first weeks, while the new branch's staff are still learning the routine.

What changes in staff and booking across two branches?

The paperwork gets the branch registered. Staff and booking get it actually running without double-booking a doctor or losing a patient's history between the two addresses.

Before you open the doors at branch two

  • One shared appointment list, not two separate registers.

    So a patient can't be booked into the same doctor at both branches at the same time.

  • Front desk at each branch knows which doctors sit where, and on which days.

  • A clear rule for moving a patient's file or history between the two branches.

  • New staff briefed on your actual fee list and no-show policy, not left to guess one.

  • One named person patients can reach if the new branch's number doesn't get answered yet.

    Common in the first two or three weeks while the routine settles.

What has to go on the fee board and the website for branch two?

The same things as branch one, at the new address. Fee display isn't optional: NMC regulation 3.7.1 (opens in a new tab) requires every doctor to display fees on the board of their chamber or the hospitals they visit, wherever they practise, and that applies at the new branch from day one.

Your website needs a page for the new branch too, with its own address, hours and phone number, rather than pointing every branch at the same generic page. The doctor's registration number goes on that page and on the fee board, unchanged from branch one.

Two branches under the same brand and the same doctor are still two premises, so treat their fee lists as two separate facts to keep straight, not one. If a procedure costs a different amount at each branch, or one branch offers a service the other doesn't, say so on that branch's own board and its own website page, rather than posting one fee list and hoping it reads as close enough for both.

What does a 60-day plan for opening branch two look like?

Three tracks running at the same time work better than one long list done in order: the paperwork, the online presence, and getting the branch staffed and briefed. Verification and approvals in the first two tracks take time you don't control, so start them early and use the middle weeks for the third.

  1. Step 1: Days 1 to 15: file the paperwork

Does a second clinic branch need its own registration under the Clinical Establishments Act?

Only if your state adopted the central Act. As of the date checked, the official portal lists Arunachal Pradesh, Assam, Bihar, Haryana, Himachal Pradesh, Jharkhand, Mizoram, Rajasthan, Sikkim, Telangana, Uttar Pradesh, Uttarakhand and every union territory except the NCT of Delhi as having adopted it. States that didn't adopt it run their own clinical establishment or nursing home law, and each branch, at its own address, needs its own registration under whichever law applies. Can one Google Business Profile list two clinic branches?

No. Google's guidelines call for one profile per real business location, and a profile is expected to have a phone number and website that represent that specific address. Branch two needs its own listing, verified separately, even if it shares a name and owner with branch one.

Does the new branch need its own biomedical waste authorisation?

Yes. Under the Bio-Medical Waste Management Rules, 2016, every occupier handling biomedical waste needs authorisation from the State Pollution Control Board or Pollution Control Committee, regardless of how much waste the facility generates. That authorisation is tied to the premises generating the waste, so a second branch needs its own.

Do we need to add the new branch to our GST registration?

If your clinic is already GST-registered, for example because you run a pharmacy counter or cross the threshold on taxable supplies, a branch in the same state has to be added as an additional place of business. This is a core amendment under rule 19 of the CGST Rules, filed in Form GST REG-14, and the rule requires it within 15 days of the change. A branch in a different state needs a fresh registration, not an amendment.

Does the doctor need a new registration number for the new branch?

No. A doctor's registration with their State Medical Council is personal, not tied to one address, so the same number carries over. It still has to be displayed at the new branch, on the same items, the board, prescriptions, certificates and receipts, as it is at branch one.

Do both branches have to charge the same fees?

There's no rule requiring identical fees, but whatever a branch actually charges has to be displayed accurately on that branch's own board and its own website page. If the two branches genuinely differ on price or the services on offer, show each branch its own real numbers rather than one shared fee list for both.
