The WhatsApp Business app is free and works well for a clinic run by one person off one phone. None of this is a flaw in the app. It's built for one owner running the account from a phone, and it does that well. The trouble starts when a second doctor, a second branch, or a front desk that wants to reply from a computer joins in, since the app has no idea of separate logins for separate people, only linked devices on the same account.

What the WhatsApp Business Platform adds

The WhatsApp Business Platform, which Meta also calls the API, is built for clinics and businesses that outgrow the app. Inside an open chat, a reply to a patient who messaged you within the last 24 hours works the same as it does on the free app. What the Platform adds is a way to message outside that window: a message template, written in advance and approved by Meta, which is the only kind of message the Platform can send once the 24-hour window has closed or before a patient has written in at all.

Templates cover three kinds of message: utility (a confirmation or an update about something the patient already asked for), authentication (an OTP or a verification code), and marketing. Beyond templates, the Platform itself has no phone app or screen. A clinic reaches it through a solution provider, who builds the inbox, the automation and, usually, the ability for more than one staff member to reply at the same time.

Approved display names and the green tick, explained

Two Platform-only ideas cause most of the confusion: the display name, and the green tick.

A clinic on the Platform sends templates under a business account that starts with a template cap: if the parent business portfolio is unverified, each WhatsApp Business Account is limited to 250 message templates. Complete Meta's business verification and get a display name approved, and the cap rises to 6,000 templates. The approved display name is simply the clinic's name as it will show to patients, checked once by Meta before templates under it can send.

The green tick is older, and mostly retired. WhatsApp used to hand a green checkmark to a small number of well-known official business accounts, based on notability rather than payment. That system has since been folded into Meta Verified, a paid subscription that adds a checkmark badge, impersonation protection and more linked devices, in tiers from Business Standard upward. A business that already carried the green tick keeps its verified status with no action needed. A clinic wanting the badge today applies through Meta Verified, on either the app or the Platform, not through the older free route.

What each one actually costs

The WhatsApp Business app itself stays free, no matter how the clinic uses it. The Platform has no signup fee from Meta either, but Meta has charged per delivered template message since 1 July 2025, and the provider connecting your clinic to the Platform usually charges its own fee on top of Meta's.

What happens if you move your clinic's number to the Platform

A number already running on the free app can move to the Platform, but the two routes end very differently for the clinic's existing chats.

  1. Step 1: Back up the app's chat history first

  2. Step 2: Choose a route

    The direct route deletes the number's account and re-registers it on the Platform. A newer route, coexistence, keeps the WhatsApp Business app and the Platform running on the same number together.

Either way, the clinic doesn't register with Meta directly. A solution provider handles the technical setup, the display name approval, and, usually, the monthly bill.

Which one fits your clinic

There's no rule that says a clinic must move once it reaches a certain number of patients. What usually forces the decision is people, not volume: how many staff need to reply at the same time, and whether the clinic regularly needs to message someone who hasn't written in first.
