How do you increase patients in a physiotherapy clinic in India?

Medslots team, Letex Media Co.How we write and check facts

Updated 8 min readFacts checked

Short answer

Most physiotherapy clinics already have the patients they need, they just lose them along the way. Reply fast to WhatsApp and referral enquiries, keep your Google profile accurate, ask every patient for a review the same way, and follow up anyone who stops a treatment plan early. NCAHP's draft ethics code limits how you can advertise.

Key takeaways

  • Most growth sits in patients you already have some contact with: enquiries, doctor referrals and patients who stopped a treatment plan early.
  • Doctor referrals should never involve a fee. The physiotherapists' own ethical rules and the NMC's rule for doctors (6.4.1) both bar paying or taking a commission for referring a patient.
  • Following up a patient who stopped treatment early is continuity of care, not a sales message, as long as it names the plan and carries no offer.
  • NCAHP's 2026 draft ethics code for physiotherapists is not yet notified, but it already limits solicitation, patient images and paid social media reach.

Physiotherapy has a growth problem most clinics never measure: patients who start a treatment plan and never finish it. Before you spend on ads, count what you are already losing.

Where does a physiotherapy clinic lose patients it already has?

Mostly between the first message and the last session of a plan. A patient referred by their orthopaedist who hears nothing back for a day may not come in at all, and a patient who improves after four sessions of an eight-session plan may quietly stop and you never find out why.

There is no reliable India-wide figure for how many patients a physiotherapy clinic loses this way, so don't trust anyone who quotes one. Follow a patient through these six steps and see where yours leaks.

  1. Step 1: Referral or enquiry

Leaks when a referred patient's message sits unanswered

A doctor's note, a WhatsApp message, or a Google Maps tap.
  2. Step 2: Gets a reply

Leaks when the fee question gets no clear answer

The fee, what the first session involves, and a slot.
  3. Step 3: Books an assessment

Leaks when nobody confirms the slot

A clear day and time the patient agrees to.
  4. Step 4: Assessment sets a plan

Leaks when the number of sessions isn't explained upfront

The physiotherapist examines and explains the sessions needed and the cost.
  5. Step 5: Completes the plan, or stops early

Leaks when nobody checks in after a missed session

Most patients who stop don't tell the clinic why.
  6. Step 6: Leaves a review

    Leaks when nobody asks

If the patient chooses to, after being asked like everyone else.

Do fee questions on WhatsApp get a range, not "please visit the clinic"?Yes / NoWrite down a range for your common assessments and packages, and share it.
Are patients who miss two sessions in a row contacted?Yes / NoKeep a simple list of active plans and who hasn't shown up.
Does every referring doctor hear how their patient is progressing?Yes / NoSend a short update after the assessment and again at discharge.
Is every patient asked for a review the same way?Yes / NoUse one review link or QR code for everyone, with no incentive.

How do you get more doctor referrals for physiotherapy?

Through communication and results, not through paying for them. Most physiotherapy patients arrive on a doctor's advice, after surgery, an injury, a stroke, or a back or joint problem, so orthopaedists, neurologists and general physicians are often your biggest source of new patients.

  • Send a short update. After the assessment, and again when the patient improves or is discharged, tell the referring doctor what you found and did. It costs a few minutes and it is the single biggest reason a doctor keeps referring to the same clinic.
  • Reply to the patient the same day. A patient who was told "go see a physiotherapist" and gets no reply for two days often doesn't go at all, and the doctor never learns why.
  • Never offer or accept a fee for the referral itself. This is not a courtesy question, it is a rule on both sides of the relationship. See the callout below.

What do you do when a patient stops a treatment plan early?

Message them once, treat it as continuing their care, and never turn it into a sales pitch. This is different from a dental or eye check-up recall: the patient started something specific, and there is a clinical reason to find out whether they need to finish it.

Stopping early is common in physiotherapy generally. A 2023 international systematic review in Frontiers in Sports and Active Living (opens in a new tab), covering 57 studies, notes that non-adherence to exercise therapy often exceeds 50% in patients with chronic conditions. That review looked at exercise programmes worldwide, not clinic visits in India specifically, so treat it as a reason to check your own numbers rather than a figure to quote about your clinic.

Before you message a patient who stopped treatment early

  • The patient agreed to hear from the clinic.

WhatsApp's Business Messaging Policy requires opt-in permission before you message someone.

  • The plan is genuinely incomplete, based on what the physiotherapist assessed.

  • The message names the plan, not an offer or a discount.

  • It explains how to stop these messages, and you honour every stop request.

The same policy says to respect requests to opt out, on or off WhatsApp.

  • No mention of the condition or diagnosis that someone else reading the phone should not see.

Sample wording for messages like this is in WhatsApp recall messages for past patients. Rules source: WhatsApp Business Messaging Policy (opens in a new tab).

What should a physiotherapy clinic reply to on WhatsApp?

Fee questions and referral messages, the same day, with no clinical advice given over chat. A patient messaging with pain or swelling needs to be seen, not diagnosed on WhatsApp.

What should a physiotherapy clinic fix on Google?

Claim the profile, use the clinic's real name, and keep hours and services correct. Google says local results depend mainly on relevance, distance and prominence, and that businesses with complete and accurate information are more likely to show up (opens in a new tab).

The full walkthrough is in the guide to Google Business Profile for doctors.

How do you ask physiotherapy patients for Google reviews?

Ask every patient, the same way, with no reward, typically once they finish or are discharged from a plan. Google suggests sharing a review link or a QR code, and its policy lets businesses ask for reviews of genuine experiences.

When you reply to a review, don't confirm that the person is your patient or name their condition. The IAP's ethical rules say physiotherapists shall maintain secrecy of the patient's disease and shall not divulge it to anyone outside professional discussion.

What does the NCAHP Act mean for physiotherapy marketing?

It means a dedicated regulator, separate from the NMC code doctors follow, and a draft ethics code that already limits solicitation and social media claims even though it isn't final yet. The National Commission for Allied and Healthcare Professions Act, 2021 (opens in a new tab) lists "Physiotherapist" as a designated Category 3 profession, so physiotherapy clinics fall under it, not under the doctors' code.

Does one extra patient cover the cost?

It depends on your own fee and costs, so work it out rather than trust anyone's promise. Type what you charge for a session and what that session costs you.

Calculator

How many sessions a month cover ₹4,999?

Arithmetic on your own fee, not a promise. ₹4,999 is the Medslots monthly price for one clinic.

Consumables, equipment time. Type 0 if you want to leave costs out.

Sessions a month

Enter your numbers above to see this.

How this is worked out: Sessions = ₹4,999 divided by (your fee minus your cost for the session), rounded up. If the cost is equal to or more than the fee, there is no result. Nothing you type here is saved or sent anywhere.

If the answer is a handful of sessions, the question becomes simple: are you currently losing that many to slow replies, an unanswered referral, or a plan nobody followed up on? The self-check above tells you. For more on running the front desk, browse the Medslots guides for clinic owners.

Common questions

How can a physiotherapy clinic in India get more patients?

Start with people who already know about you. Reply fast to WhatsApp and referral enquiries, keep your Google Business Profile accurate, ask every patient for a review the same way, and follow up patients who stop a treatment plan early. Build doctor referrals through communication and outcomes, not payment. Then consider new marketing.

Can physiotherapists advertise their clinic in India?

NCAHP's draft ethics code for allied health professionals, opened for public feedback in April 2026 and not yet notified as of 17 Sep 2026, allows limited advertising such as announcing a new practice, but bars soliciting patients directly or indirectly. Check for the final notified version before planning a campaign. This is not legal advice.

Is it okay to pay a doctor for referring patients to my physiotherapy clinic?

No. The Indian Association of Physiotherapists' ethical rules say physiotherapists shall not solicit patients through fee splitting, and the NMC's Code of Medical Ethics Regulations, 2002 (clause 6.4.1) bars a doctor from giving, soliciting or receiving any gift, commission or bonus for referring a patient. Build referrals through good outcomes and clear communication instead.

How do I follow up with a patient who stopped physiotherapy mid-treatment?

Message them once, naming the plan the physiotherapist set and asking if they would like to continue, with no discount or offer attached. Only message patients who agreed to hear from the clinic, and include a way to opt out. Treat it as continuity of care, not a sales message.

Physiotherapy clinic me patient kaise badhaye?

Jo patients pehle se contact kar rahe hain unhe sambhaliye: WhatsApp par jaldi reply kijiye, Google profile sahi rakhiye, har patient se ek hi tarah review maangiye, aur jo patient beech mein treatment chhod dete hain unhe ek baar follow up kijiye. Doctor referrals paise se nahi, achhe results aur communication se banaiye.
